Final Workshop Registration
Online registration is preferred. However, if you'd like to mail in your final registration and payment with a check, please complete this online registration and select "Pay by check" instead of entering credit card details. Print out your registration receipt and mail it with a check in U.S. dollars made out to Discalced, Inc.:

Registration deadlines: Final registration and payment, whether online or by mail, must be received in full no later than 15 days before the start date of the workshop. Registrations received after this time may be accepted on a case-by-case basis, but are not guaranteed.

Cancellation policy: Participants who cancel before April 15, 2024 will receive a full refund less a $20 administrative fee. Participants who cancel on or after April 15 will not receive a refund, but will receive a credit (minus $20) for use at a future workshop. Participants who test positive for COVID-19 within 5 days of the mini-symposium beginning, and show proof of a positive test, will receive a full refund or full credit. Cancellations must be communicated via email to support@danceforpd.org.

Credits: Attendance at this workshop earns you 8 credits that you can apply toward establishing eligibility for or maintaining Dance for PD certification.
Affiliation: Dance for PD is a registered trademarks and may not be used to describe any classes without official licensing. Participation in this workshop is for training purposes only, and does not license you to use these trademarks.
Photo/video release: By registering, I hereby allow Dance for PD, Mark Morris Dance Group, Canada's National Ballet School, and Dancing with Parkinson's Canada to publish photographs or videos taken of me, and my name, for use in printed Dance for PD publications and websites, as well as in publications and websites published by Dance for PD workshop hosts and presenting partners. In consideration for my voluntary participation in publications produced by Dance for PD, or any collaborator, I agree that I will receive no financial compensation. Furthermore, I agree that participation in any print or online publication produced by Dance for PD, Mark Morris Dance Group, Canada's National Ballet School, and Dancing with Parkinson's Canada, or any outside third party interests, confers no ownership or special rights whatsoever. I release the Dance for PD, Mark Morris Dance Group, Canada's National Ballet School, and Dancing with Parkinson's Canada for any claims of liability by me in connection with my participation in the production of these photographs.
Waiver: I understand that participating in the workshop may require mental judgment and a high degree of physical fitness, agility, stamina and strength, and that this may include strenuous exercise. I agree to assume full responsibility for any risks, injuries or damages, known or unknown, which I may incur as a result of participating in the workshop. I, individually, and on behalf of my heirs, successors, assigns and personal representatives, hereby agree to discharge, release, and hold harmless the Dance for PD, Mark Morris Dance Group, Canada's National Ballet School, and Dancing with Parkinson's Canada, their respective board members, trustees, officers, employees, agents and independent contractors, from any and all liability whatsoever for any and all damages, losses, or injuries (including death) I sustain to my person or property or both, including but not limited to any claims, demands, actions, causes of action, judgments, expenses and costs, including attorney’s fees, which arise out of, result from, occurring during, or are connected in any manner with my participation in the Workshop.
